What was the Connected Care Partnership?

We worked together with other health and social care providers in Birmingham and Sandwell on an NHS England funded programme to develop a consistent, joined-up service that puts patient wellbeing at its heart. Known as the Connected Care Partnership; made up of a group of organisations including Modality Partnership, Intelligent Commissioning Federation, Sandwell and West Birmingham NHS Hospitals Trust, Birmingham Community NHS Foundation Trust, Birmingham and Solihull Mental Health NHS Foundation Trust and worked closely with Sandwell and West Birmingham Clinical Commissioning Group.

What was an NHS Vanguard site?

We were selected by NHS England as one of the 14 test sites known as a Vanguard site. Vanguards were set up to test out new models of care, with the aim of creating efficiencies and providing holistic care that prevents future population health issues and to help make the NHS sustainable and fit for the future. The types of projects we tested out included:

Care Families – enhancing primary care

In Birmingham we changed the way our staff worked and found new ways to make access to care easier, more seamless and of the highest quality. Our GP practices were organised into care families. Each care family cared for a certain group of patients. Making it more likely for patients to see the same doctor, nurse and even receptionist in their practice. By doing this we aim to improve continuity of care, and also help to make sure patients are seen by the right person, in the right place at the right time.

Care when you need it and closer to home

We have increased the number of specialised services that are delivered within GP practices and closer to people’s homes. This includes the types of services that traditionally would have meant a hospital trip such as Cardiology, Gynaecology, ENT, X-ray, Dermatology, Rheumatology, Orthopaedics, Opthalmology to name a few. So patients spend less time making trips to hospital and will only go there if they need to. By providing more outpatient services in the community we have reduced waiting times and increased efficiency in the local healthcare system.

By being part of the Connected Care Partnership, Modality Partnership have worked across boundaries and continue to be commited to providing an excellent patient experience. As an organisation we have shared these learnings across all of our regions to help spread new ways of working and support the future of the NHS. The programme was independently evaluated by the University of Birmingham’s Health Services Management Centre (HSMC), in collaboration with The Nuffield Trust.
